What Hong Kong HR leaders told us about benefits in Q4 2026
Findings from a Hong Kong employee health and wellbeing event in October 2026: what HR and reward professionals want from their next benefits renewal, and where provision falls short.
premium costs
benefits platform
wellness allowance
Key takeaways
- Cost dominates the renewal agenda 74% named reducing overall premium costs as a renewal goal. No other goal comes close.
- Wellness is the clear second goal 48% want better wellness and mental health support, and 36% want cost cuts and better wellness together. Only 35% offer an EAP or mental health benefit.
- The flexibility gap is the widest in the survey 71% have or want an FSA or wellness allowance, yet only 13% run a flexible benefits platform. 44% are interested in one, a gap of +31pp.
- Group medical is under review 96% already offer group medical, and 60% of those organisations are looking at group medical solutions.
- Smaller organisations want flexibility most 84% of organisations with up to 200 employees have or want an allowance, against 59% of those with 201+. Larger organisations lean harder on cost (78% vs 70%).
- HR teams want less admin 30% want their renewal to reduce HR administrative workload.
HR and reward professionals at one event
MixCare ran this survey at its booth at a Hong Kong employee health and wellbeing event in October 2026.
Most respondents work in HR: HR, reward, total rewards, compensation and benefits, and people and culture roles. A smaller group were insurer contacts, plus a few people in sales and other roles. The sample splits almost evenly by size: 51% come from organisations with up to 200 employees and 49% from organisations with 201 or more.

Cost dominates: 74% want lower premiums
74% of respondents want their next renewal to reduce overall premium costs. No other goal comes close.
Cost is usually paired with something else. 27% picked both cost reduction and more flexibility, and 23% picked both cost reduction and engagement. HR teams want to spend less and still offer more.
"Cost is now the first question in almost every renewal conversation. The organisations that come prepared with spend and utilisation data will be the ones that negotiate from strength."
Wellness is the clear number two goal
48% want their renewal to improve wellness and mental health support, second only to cost. And 36% picked both cost reduction and better wellness, the most common pairing in the survey.
Provision has not caught up. Only 35% currently offer an EAP or mental health benefit.
"HR teams want to cut cost and improve wellness in the same renewal. Wellness spend that sits inside a capped budget is far easier to approve than a new fixed line item."
The flexibility gap: 71% have or want an allowance, 13% run a platform
44% of respondents do not offer an FSA or wellness allowance but are interested in one. Another 27% already have one, and 29% prefer a fixed plan.
want one
38% also name more employee choice as a renewal goal. Intent is well ahead of the infrastructure needed to run an allowance at scale.
"The appetite for flexibility is already there. What most organisations lack is the platform to deliver it without adding cost or admin."
Group medical is under review
Group medical insurance tops the list of solutions of interest at 60%, yet 96% of respondents already offer it. Of the organisations that have group medical, 60% said they are interested in group medical solutions.
This reads as a review signal: many HR teams are looking for better value from the plan they already run, which fits with cost as the top renewal goal. Flexible benefits is the second most popular solution at 44%.
"When almost every organisation already has group medical, interest in it is a review signal. HR teams want better value from the plan they have."
Smaller organisations want flexibility most: 84% vs 59%
84% of organisations with up to 200 employees have or want an FSA or wellness allowance, against 59% of those with 201 or more. That runs against the common assumption that flexible benefits is a big-company concern.
Larger organisations lean harder on cost (78% vs 70%), while smaller ones put more weight on wellness (51% vs 44%).
"Smaller organisations compete for talent against bigger budgets. An allowance lets them offer real choice with a fixed, predictable cost per head."
HR teams want less admin
30% of respondents want their renewal to reduce HR administrative workload, making it the fourth most common goal, just ahead of engagement and retention (29%). The pressure is the same in large and small organisations.
This links back to the flexibility gap. Without the right tooling, running an allowance, claims and several providers adds work for HR, which helps explain why provision trails demand.
"More choice for employees should not mean more work for HR. Three in ten respondents want the renewal to take work off their plate."
Flexible benefits shows the biggest have vs want gap
Only 13% run a flexible benefits platform, while 44% are interested in one, a gap of +31pp. Gym and fitness (19% have, 25% interested) is the only other category where interest exceeds provision.
Elsewhere, interest is lowest where provision is already high, such as dental (69% have it). On average, organisations offer 4.6 benefits.
| Benefit | Have now | Interested in | Gap | Visual comparison |
|---|---|---|---|---|
| Flexible Benefits Platform | 13% | 44% | +31pp | |
| Gym & Fitness | 19% | 25% | +6pp | |
| AI Training | 21% | 13% | -8pp | |
| Engagement Events & Workshops | 35% | 26% | -9pp | |
| EAP / Mental Health | 35% | 20% | -15pp | |
| Maternity | 26% | 7% | -19pp | |
| Vaccination | 40% | 10% | -30pp | |
| Body Check | 56% | 21% | -35pp | |
| Group Medical Insurance | 96% | 60% | -36pp | |
| Dependent Coverage | 43% | 5% | -38pp | |
| Dental | 69% | 18% | -51pp | |
| Self-Funded Outpatient | n/a | 15% | n/a |
Self-funded outpatient was asked only as a solution of interest, not as a current benefit.
"The core benefits are in place. The gap is in how they are delivered: employees want choice, and only a small minority of employers have the platform to offer it."
Larger organisations lean on cost, smaller ones on flexibility
| Measure | 201+ employees | Up to 200 |
|---|---|---|
| Reducing overall premium costs | 78% | 70% |
| Improving wellness & mental health | 44% | 51% |
| Increasing choice / flexibility (FSA) | 39% | 37% |
| Reducing HR admin workload | 29% | 30% |
| Driving engagement & retention | 32% | 26% |
| Have or want an FSA / wellness allowance | 59% | 84% |
Cost leads in both groups, with larger organisations at 78%. The sharpest difference is appetite for an allowance: 84% of smaller organisations against 59% of larger ones. Stated flexibility goals are close (39% and 37%), and admin pressure is level at around 30%.
What this means for your 2027 renewal
If you are preparing a renewal brief, these results point to four practical questions to put on the table.
Benchmark before you renew
60% of organisations with group medical are already looking at alternatives, and 74% want lower premiums. Know your utilisation and claims data so you can test the market from a position of strength.Treat flexibility as a cost lever
An allowance caps spend per employee while letting people choose what matters to them. 71% have or want one, rising to 84% in organisations with up to 200 employees, yet only 13% run a platform.Fund wellness inside the budget
36% want cost cuts and better wellness together. Wellness options inside a flexible allowance can meet that demand without adding a new fixed line item.Build in real-time visibility
30% want less admin. Choose a setup with one view of enrolment, claims and spend, so more choice does not mean more spreadsheets.About this survey
