Self-Funded Outpatient vs Group Medical: A Hong Kong Employer's 2026 Guide
With APAC medical inflation at 11.3%, the gap between what employers pay in premiums and what employees actually use is significant. This guide breaks down how self-funded outpatient benefits compare to group medical cover on cost, flexibility, and utilisation, with a side-by-side comparison, real cost modelling for HK employers, and a summary of the flexible spending wallet model that HR leaders across the region are adopting ahead of Q3 renewals. HR leaders who have made the switch are recovering an estimated 12 to 22% on outpatient spend.

APAC medical inflation hits 11.3%, the highest of any global region
The Aon 2026 Global Medical Trend Rates Report confirms APAC medical inflation at 11.3%, which is 3.2 times the regional consumer price index. For employers renewing group medical in the May to July window, this translates into double-digit premium increases with no change to plan design. Inpatient claims costs are up an average of 18% across HK and SG employer pools. The core issue: premiums rise regardless of how many employees actually use the plan. HR leaders who audit utilisation before renewal and consider self-funded outpatient alternatives are recovering an estimated 12 to 22% on outpatient spend.

HK statutory minimum wage rises to HK$43.10 from 1 May: three compliance steps for HR teams
Hong Kong's statutory minimum wage (SMW) increased to HK$43.10 per hour on 1 May 2026. Employers with hourly-paid, part-time, or contracted workers under the Employment Ordinance are required to comply from this date. Three immediate actions: (1) audit all affected roles against the new rate, including contracted roles where your organisation is the principal employer; (2) update employment contracts, offer letters, and payroll rate cards; (3) brief people managers before they field questions from affected staff. The SMW increase compresses the total compensation ratio for lower-wage workers. Factor this into your Q2 to Q3 compensation review.
